Examination Matters

 

Resetting of Personal Identification Number (PIN)

Students who have forgotten their PIN can reset their PIN online at the Student Portal. Instructions on the resetting of PIN are given. Alternatively, students can request a reset of PIN at the Student Services Centre.

 

 

Statement of Results

Assessment results are published online at the Student Portal. Students are encouraged to print a copy of their statement of results from the Student Portal for their records. Statement of results will not be mailed to students.

 

 

Academic Transcript

The academic transcript contains all assessment results that have been published to-date.

 

Students may print their unofficial academic transcript from the Student Portal. The official academic transcript will be issued only by the Registrar.

 

Graduates will receive a complimentary copy of their academic transcript, through post to their term address, within seven (7) working days upon release of their final set of results.

 

 

CCA Transcript (for Graduates only)

The CCA transcript is available only to graduates. Graduates will receive a complimentary copy of their CCA transcript, through post to their term address, within four (4) to six (6) weeks upon successful completion of their course of study.

 

 

Request for Academic Transcript / CCA Transcript

Students can request an issue of their academic transcript anytime during their course of study and after they have graduated. The CCA transcript is, however, available only to graduates. An administrative fee of S$5.35 per transcript will be levied.

 

Request for the academic transcript / CCA transcript may be made at the Student Services Centre, personally or through a proxy. A copy of the prescribed form is available here.

 

To make a request through a proxy, students/graduates must give their consent with an authorisation letter and give their proxy’s name and identity card number. The proxy must produce the letter of authorisation and his/her identity card at the Student Services Centre.

 

Current students and those who have left recently (i.e. within six (6) months) can also make their request online at the Student Portal. Graduates can also make their request online through the Alumni Portal. Payment for online requests must be made via eNETS Debit for internet banking account holders with DBS Bank, POSBank, UOB or CitiBank N.A.

 

Requests that are made at the Student Services Centre will be processed there and then. Transcripts that are requested online will be mailed to the mailing address, given in the request form, within seven (7) working days of the request.

 

 

Letter of Certification for Successful Completion of Course (for Graduates Only)

Graduates will receive a letter of certification of their qualification, together with a complimentary copy of their academic transcript, through post to their term address, within seven (7) working days upon release of their final set of results.

 

 

Request for Letter of Certification for Successful Completion of Course (for Graduates Only)

For a re-issue of the letter, graduates can request a similar letter of certification at the Student Services Centre. Recent graduates (i.e. within six (6) months of conferment of qualification) can also make their request online at the Student Portal. The letter will be mailed to the mailing address, given in the request form, within seven (7) working days of the request.

 

 

Collection of Certificate

Graduates will, generally, be presented with their certificate at the Graduation Ceremony. Graduates who did not attend the Graduation Ceremony, may collect their certificate at the Student Services Centre, after seven (7) working days from the last day of the Graduation Ceremony. Graduates may either collect their certificate personally or authorise a proxy to do so on their behalf. They must give their proxy’s name and identity card number in their letter of authorisation. The proxy must then produce the letter of authorisation and his/her identity card at the Student Services Centre.

 

Graduates are encouraged to collect their certificate promptly. While the Polytechnic makes every effort to safe keep uncollected certificates, it will not be responsible for the loss or damage of any uncollected certificates.

 

 

Request for Replacement of Certificate

Graduates can request a replacement of their certificate. Request for replacement certificate must be made on prescribed forms available at the Student Services Centre. A copy of the form is also available here. An administrative fee of $21.40 per replacement certificate will be levied.

 

The replacement certificate will be ready after eight (8) working days from the day of request. Graduates may either collect their replacement certificate personally at the Student Services Centre or authorise a proxy to do so on their behalf. They must give their proxy’s name and identity card number in their letter of authorisation. The proxy must then produce the letter of authorisation and his/her identity card at the Student Services Centre.
